The Christian Basketball
Association (CBA) is a non-profit, voluntary participation, church basketball
league. All participants and/or their parents/legal guardians accept full
liability in regards to any and all injuries that may occur while practicing, playing in games at Ebenezer ARP, Neelys Creek
ARP, First ARP, New Kirk Baptist, Mt Holly UMC, Oakdale Baptist, St. Johns UMC and Woodland UMC, and traveling related
to their participation. The undersigned have read this waiver and agree to
their responsibility and risk. The undersigned will not hold the church they
are representing, the church location where the games are played, or the CBA
board and/or director for injuries associated with their participation.
